Required Documents for Athlete Membership

Athletes who are 17 and under: 1
  • Membership form (signed and dated by legal parent and professor)
  • 3×4 Color picture of the athlete (passport-style, in front of a plain white background)
  • Copy of the athlete’s legal government issued ID (Birth Certificate, Passport, etc.)
  • Copy of the parent / legal guardian’s government-issued photo ID (who is signing the application)
Athletes who are 18 or older: (White, Blue and Purple belts) 1
  • Membership form (signed and dated by the athlete and professor)
  • 3×4 Color picture of the athlete (passport-style, in front of a plain white background)
  • Copy of the athlete’s government-issued photo ID (Passport, Drivers License, etc.)
Athletes who are Brown and Black Belt: 1
  • Membership form (signed and dated by the athlete and professor)
  • 3×4 Color picture of the athlete (passport-style, in front of a plain white background)
  • Copy of the athlete’s government-issued photo ID (Passport, Drivers License, etc.)
  • Belt Records (Document available to the athlete after membership request.)
All Belts – Membership Renewal: 1
  • Membership form (signed and dated by the athlete and professor)
  • ** 3×4 Color picture of the athlete (passport-style, in front of a plain white background)
    • Ages 4 to 17: Requested every year
    • Ages 18 and up: Requested every 5 Years
  • ** Copy of the athlete’s government-issued photo ID (Passport, Drivers License, etc.)
    • Requested every 5 years
    • Requested whenever changing federations (i.e.: from IBJJF to USBJJF)
*Attention Brown and Black belts:
  • Athletes who are registering for the first time and are applying as Brown or Black Belts will be evaluated based on the Provisional Graduation System laid out in article 7 of the IBJJF graduation system.

  1. 1) Please note: We reserve the rights to request for background check from any athlete in any type of application at our discretion. In the case of a requested background check comes back with information that could lead to a disqualification, we reserve the right to deny the membership application.
